DOCTOR APPOINTMENT
RESCHEDULE AND CANCELLATION POLICY

Your appointment is very important to us and it is reserved especially for you. We realize that there can be some unexpected things that can come up in our lives. While truly sympathetic, when a client cancels without giving enough notice, they prevent another client from being seen by our physicians. In fairness to all clients as well as to all doctors, this policy is in effect regardless of the reason for the cancellation. Fertility Cloud has a minimum 24 hour cancellation/rescheduling policy. If an appointment is missed, canceled or changed with less than 24 hours notice, the client will be charged a cancellation fee of 100% of the service charge and there will be no refunds. Repeated cancellations may incur a $100 administrative fee and result in loss of access to our services.

HERE'S HOW IT WORKS:

  • We require a debit/credit card to hold your scheduled reservation
  • You can reschedule or cancel your appointment free of charge, with minimum 24 hour notice prior to your appointment
  • If you cancel or miss your scheduled appointment, no refunds will be provided
  • Attempt to change your appointment within 24 hours, your reserved appointment will be considered a cancellation and the cancellation policy will be applied
  • "No shows" are considered a cancellation and the cancellation policy will be applied
  • If Fertility Cloud has to cancel/reschedule your reservation, you can choose to rebook for another time or accept a full refund.

REMINDER EMAIL & TEXT

As a courtesy, we will send multiple email and text reminders about your reserved appointment 24 hours prior to your appointment. However, if we are unable to reach you, please understand that it is your responsibility to remember your appointment dates and times to avoid late arrivals, missed appointments and the cancellation fee.

HOW TO RESCHEDULE?

The change of time and date can be done by canceling your existing appointment and scheduling a new appointment via our online booking system (https://live.vcita.com/site/fertilitycloud). Online cancellation is only possible outside of 24 hours prior to your scheduled appointment. Alternatively, you can contact Fertility Cloud directly and request to reschedule your appointment by email at office@myfertilitycloud.com or by phone/SMS at 707-361-6339. Should you be directed to voicemail, a message left detailing your request to reschedule prior to the 24 hour deadline will be honored.

HOW TO CANCEL?

Cancellations more than 24 hours prior to the scheduled appointment can be done using our online booking system (https://live.vcita.com/site/fertilitycloud). Alternatively, you can contact Fertility Cloud directly and request to cancel your appointment by email at office@myfertilitycloud.com or by phone/SMS at 707-361-6339. Should you be directed to voicemail, a message left detailing your cancellation prior to the 24 hour deadline will be honored. Less than 24 hours cancellations will be considered last minute and you will be charged the full amount of the reserved service appointment booking.

HOME TEST KIT
REFUND POLICY

When we order on your behalf a home test kit, it is typically shipped the next business day from one of our partner warehouses using UPS, USPS or FedEx shipping services. Once we've transmitted your order, it is out of our control and being fulfilled by another company.

HOW TO CANCEL?

If you need to cancel your home test kit order, contact Fertility Cloud directly and request to cancel your order by email at office@myfertilitycloud.com or by phone/SMS at 707-361-6339. We will reach out to the vendor and ask for a cancellation. If they have already shipped the order - no cancellation or refund is possible. If the vendor will be able to cancel the order we will provide a full refund.

Rx MEDICATION
CHANGE AND REFUND POLICY

When we order medication on your behalf, it is typically shipped the same business day from one of our partner pharmacies using UPS, USPS or FedEx shipping services. Once we've transmitted your order, it is out of our control and being fulfilled DIRECTLY by a pharmacy. They will reach out to a client to confirm the prescription and home address.

HOW TO CANCEL?

If you need to cancel your medication order, contact Fertility Cloud directly and request to cancel your order by email at office@myfertilitycloud.com or by phone/SMS at 707-361-6339. We will reach out to the pharmacy and ask for a cancellation. If they have already shipped the order - no cancellation or refund is possible. If the pharmacy will be able to cancel the order we will provide a full refund.

PREPAID TREATMENT PLANS
REFUND POLICY

To assist with treatment costs, Fertility Cloud may offer discounted treatment plans when clients prepay for 3 or more months of treatment. The following policy applies to all prepaid plans:

  • If a client becomes pregnant before the end of the plan, and pregnancy is confirmed by lab work, all unused funds will be returned on a prorated basis.
  • If a client decides to take a break from our treatment but continues to seek support from the office, they will be charged a monthly Fertility Care Monitoring fee according to a published pricing schedule https://myfertilitycloud.com/services-and-pricing
  • All prepaid 3-month plans must be consumed within 6 months from the date of payment.
  • All prepaid 6-month plans must be consumed within 12 months from the date of payment.
  • Unused funds can be transferred to Premium Prenatal Care or Premium OB/GYN Care programs.

HOW TO CANCEL?

If you need to cancel your treatment with us, contact Fertility Cloud directly and request to cancel your order by email at office@myfertilitycloud.com or by phone/SMS at 707-361-6339. All refunds will be issued to the original form of payment and may take up to 7 business days to reflect on the account.
